Interactive Forms - Stat Dec & Affidavits

The following interactive resources are provided for use by Honorary Justices when witnessing documents with a simple format that is easy to read and use. The interactive layout of the forms makes it easier for clients to identify where to place information and fill it out as needed.  These forms work well with remote witnessing with clients and enables them to be completed quickly.  The interactive forms provided are Statutory Declaration, plus Certificate Page for Exhibits; Affidavit plus Certificate Page for Annexures and Commonwealth Stat Dec form.  The association is providing these as a download that you can use as and when needed…

The revised Oaths & Affirmations 2018 Act became effective from the 1st March 2019. There have been a number of changes in reference to what is now mandatory or prescribed in regards to wording for swearing as well as for various forms. Powers Of Attorney - Advanced Care Directive - OPA Booklet - Forms

The revised Powers Of Attorney Act 2015 & 2017 Amendment  brought a number of changes to the Enduring Powers Of Attorney (EPOA) as well as additional classifications on the type of powers that can be given, while at the same time protecting all at the same time.

As part of the ongoing change to update the Enduring Medical Powers of Attorney  was replaced by the Advanced Care Directive (ACD)  which was effective from March 2018. This was a major change as to how people can handle their medical treatment when they are not able to. We recommend to review the attached resources below for education purposes.

Take Control Booklet & EPOA / ACD Blank Forms

OPA (Office of Public Advocate) created a information booklet called Take Control which provides detailed information on EPOA & ACD for use by the public and also providing information as to what witnesses are needed and should do.  The most recent (2021) Take Control booklets no long have the blank forms for these items included,  they are only available as a separate download from the OPA site.
